Research evidence indicates the incidence of errors in prescribing, preparing and administering injectable medicines is higher than for other forms of medicine. Unlabelled syringes pose a significant risk when products are prepared in clinical areas therefore appropriate labelling of syringes is an important requirement for the safe administration of injectable medicines.
In its “Draft Guidance for Industry on Safety Considerations for Container Labels and Carton Labelling Design to Minimize Medication Errors the US FDA recommends the use of a transferable or peel-off label for the commercial container of injectable products. This type of label can help minimize the use of unlabelled syringes because the label would be attached to the commercial container closure, present at the point of product preparation, and not be discarded as in the case with other auxiliary labels often provided with the carton. This is based upon the observations by the FDA that “once an injectable medication is withdrawn from the commercial container closure (e.g. vial or ampoule) into a syringe for administration, the syringe no longer provides information needed by the end user to verify the drug name and strength prior to administration. Such unlabelled medication has led to administration of the wrong drug and wrong strength."
Step 1: peel back the upper label and turn it aside. The upper label remains attached to the vial
Step 2: remove the second label (syringe label) from the vial
Step 3: fix the syringe label onto the syringe
Step 4: draw up the solution once the syringe is labelled
